Non-urgent advice: Registration eligibility
How to register:
The practice now operates an online registration system called MiHealth Portal. If you would like to register with us and are residing in our practice boundary please follow the link below to sign up for the new patient registration service. Once you have set up a user name and password you will be able to complete all the required registration information online. If you need to register with a practice urgently and there is a reason that you are unable to complete the online process, please phone reception for further advice.
Please note:
- You can only register if you have a UK registered telephone number. If you register with a non UK telephone number your application will be rejected.
- You will only be allowed to proceed with your application if your postcode falls within our geographical boundary. The system will pop up with a warning or a ‘not valid’ message.
- We will continue to use paper forms to register new babies, your Health Visitor will help you with this.
- we may require evidence of identity to provide certain services (refer to our registration policy regarding ID)
- the practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
As your medical records may take a little time to arrive, please ensure you have at least one month’s supply of your repeat prescription medication. If needed, we may contact your previous practice to clarify any details about your medication.

Medications – Benzodiazepines and Opiates
As part of our commitment to safe prescribing, we regularly review all medications—particularly those that can be harmful or addictive, such as Diazepam, Gabapentin, Pregabalin, or Tramadol. By joining our practice, you agree to work with us to ensure your doses are appropriate, which may include guided dose reductions to protect your long-term health.
Rights and responsibilities
We strongly support the NHS policy of zero tolerance. Any patient attending the practice who abuses the doctors, staff or other patients verbally, physically or in any threatening manner whatsoever, will risk immediate removal from the practice list.

Registered with a Doctor outside the UK
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
Register online
The MiHealth Portal online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services. You must register with MiHealth Portal to use the form.
Vision Online Services
As part of the digital New Patient Registration Form, within the Patient Health Questionnaire, you will be invited to have access to Vision Online Services – Patient Services.
With a Patient Services Account you can order prescriptions online and book/cancel GP in-person appointments.
You will receive an email within 2 weeks of registration from NoReply@MyVisionOnline or [email protected]. Please check BOTH your inbox and spam for this email to complete Patient Services Account set-up.
If you leave the UK for more than 3 months
Your medical records remain with your registered GP but we will provide you with full medical care while you are in the area. If you leave the UK for a period of more than three months the doctor is no longer responsible for your medical care and may not provide repeat medication in your absence.
Immediately necessary treatment
If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from our Practice for up to 14 days. After 14 days, you will need to register as a temporary or permanent patient.
Temporary services
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the local Practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
