A note about Medical Records
When reviewing your medical records, you may sometimes see information recorded in a way that is unclear or does not fully reflect your medical history or conditions.
There are several reasons why this may occur:
- Recording errors: Information may occasionally be recorded incorrectly or dated inaccurately. If you notice something that appears to be wrong, please let us know so we can investigate and make any necessary corrections. Where possible, please raise concerns in writing rather than by telephone. Please do not book an appointment solely to discuss issues with your medical records.
- System limitations: Medical records are often coded using specific clinical terms recognised by computer systems. These systems do not cover the full range of medical conditions, particularly rare, complex, or newly identified ones. As a result, approximate terms may sometimes be used.
- Medical terminology: Clinical language may differ from everyday language, and terms may not always have the same meaning as they do in general use.
